Ohhh, yes. The guy outside of the bar is trying to convince people he is from Dragon Island, a lauded and discrete island that has dragons and swords and castles and whatnot. By 90% of the world, it's seen as a total myth and not a real place, since people who go there often don't come back.
Smoker is telling you that since that guy is out of his mind and not mentally stable, that she could probably manipulate him to do things a sane person wouldn't do. That's really it, it's just flavor text and says something about Smoker's character. Sorry for the confusion, but thanks for immersing yourself. :- )

I really liked this game, nice and short, apparently there are other mall games and I haven't played those, but I've played this and it's quite nice, although I do wonder how many endings the game has.

The Adventurer is running after me, Pipe gave me some crepes, and there's a guy inviting me to Dragon Island outside Parlor Mall. What to do next?
There is a blood trail coming out of Preggle's house, go deeper into the hole. Adventurer should not be interacted with, as per Preggle's advice.
The crepes will heal you.
Dragon Island isn't real, that guy is not well.

You just have to be careful around the Adventurer. Do not try to kill him, he will not get hurt ever. Simply move around him and survive!
